What We Do...

Triage Screenings

Our trained wellness team provides basic health evaluations on-site and in mobile units. These screenings help identify urgent medical or mental health needs, ensuring early detection and intervention.

When specialized care is required, Guardian Health and Wellness connects participants with trusted local providers, clinics, and support organizations. This ensures no one falls through the cracks.

We see health as more than the absence of illness. By working alongside programs like FreshStart, From Here Onward, and Moments of Dignity, we help create a foundation of stability—linking wellness with shelter, nourishment, and dignity.

Looking Ahead

Today, Guardian Health and Wellness operates as a mobile triage and referral service, but our vision is broader. With the support of donors, volunteers, and community partners, we aim to establish a dedicated Guardian Health & Wellness Clinic on the HOPE campus. This facility will provide consistent, accessible care while continuing to work hand-in-hand with our partner programs.
